The United Stroke Alliance Board of Directors is calling for nominations for the Marylee and John Nunley Service Award. If you are interested in nominating someone that meets the eligibility criteria for this award, please complete this nomination form. Registrations will be reviewed and selected by the Board of Director of United Stroke Alliance during their annual meeting November 5, 2022.
All nominations are due by November 1, 2022.
Marylee and John Nunley Service Award
This Award recognizes a person that demonstrates support to stroke survivors and/or provides stroke education to communities. This effort could be characterized by providing caregiving to a stroke survivor(s).
Volunteering for programs and events to assist USA or providing services to a stroke survivor in need.
Providing exceptional care while in a medical facility or being a first responder.
Providing stroke education in their community.
Nominee Qualifications
Has/have been a personal caregiver to a stroke survivor(s)
Has recognized a person having a stroke and called 911.
Has provided exceptional clinical care during a hospital stay or outpatient setting
Has provided numerous stroke education presentations in their respective communities
Has volunteered their time for programs and events to assist USA or provided personal services to a stroke survivor in need.
